There are numerous medical, dental, orthodontic and vision plans, as well as life insurance and AD&D, voluntary-supplemental life insurance, and short-term disability available through the AGC Health Plan as described below.

As an employee, you will have coverage under the Plan your employer has selected for you. If you are unsure about which Plan you have, please call 1-888-242-4357 before receiving care or submitting a claim.

Effective 1/1/2007 for employees and/or covered dependents that did not enroll in the AGC Health Plan on their hire date or the date on which the AGC Health Plan was effective for their group, those employees and/or dependents will not be eligible to enroll in the health plan throughout the year without a qualifying event as defined under federal law. The only other opportunity for enrollment will be during the Plan’s annual open enrollment period of December 1 through December 31 of each year.
